Medical Home Network

In 2009, CSEF established Medical Home Network (MHN) to address problems experienced by Medicaid-covered women and children who lack access to appropriate and coordinated healthcare services. MHN's mission is to improve the health of South Side area residents by organizing healthcare providers to enhance quality, improve access and reduce cost. Anchored by medical homes that serve as centralized points of care for acute, chronic and preventive services, MHN's goal is to serve as a model for collaborative and patient-centered care. The vision of MHN is to:

  • Establish primary care medical homes for target populations, beginning with pregnant women and children on the South Side
  • Foster clinical collaboration among public and private providers
  • Facilitate filling gaps in the continuum of care and minimize service duplication
  • Strengthen the healthcare delivery system for all medically underserved populations
